Long Term Structured Residence (LTSR)

Who does the LTSR serve?

LTSR regulations require:

  • 18 years of age or older
  • Resident of Washington or Greene County
  • Seriously mentally ill
  • Least restrictive level of care appropriate
  • Not a danger to self or others
  • What services will the LTSR provide?

Residential care includes:

  • Medical/psychiatric services
  • Counseling
  • Transportation
  • Recreational Activities/Socialization
  • Money Management
  • Vocational Assistance
  • What are the program’s objectives?
  • Stability of the resident’s psychiatric condition and
    medication regime
  • Elimination of acute psychiatric interventions
  • Attainment of individual treatment goals
  • Successful completion of trial leaves
  • Establishment of a social support system
  • Development of a plan for continuity of care

How long can I stay?

There is no time frame for the length of stay for each resident. Each client is able to move at his or her own pace in accomplishing the goals of their individual treatment plan.

How can a client be referred?

Referrals to the program can be made to the Washington County MH/MR as well as the Greene County MH/MR Administrative Offices. An admissions team reviews all referrals.

FUNDING

THE LTSR IS A PROGRAM OF THE MHA OF WASHINGTON COUNTY WHICH RECEIVES COMMUNITY HOSPITAL INTEGRATION PROJECT FUNDS FROM WASHINGTON MH/MR ADMINISTRATION.

Enhanced Personal Care Home (EPC)

EPC
Who does the EPC serve?

Criteria:

  • 18 years of age or older
  • Resident of Washington County
  • Be in treatment voluntarily
  • Not in need of inpatient hospital or skilled nursing care as certified by the MA-51
  • Complete required DPW standardized screening and assessments
  • Not pose a danger to the self or others at the time of referral and admission
  • Not be appropriate for placement in a forensic psychiatric facility

What services will the EPC provide?

Residential care provides assistance with tasks of daily living that includes:

  • Psychiatric Rehabilitation
  • Transportation
  • Shopping
  • Maintaining Appointments
  • Recreational Activities
  • Money Management
  • Socialization
  • Supervision of self-administered medications

What are the Program Expectations?

The Mental Health Association Enhanced Personal Care Home follows the holistic approach. Both personal preference and individual strengths are considered when activities are planned. The programs strives to treat all residents with dignity and respect. It adheres to the principles of the Community Support Program (CSP). The program is fair and deliberate in its handling of all resident and staff issues and concerns. It fosters inclusion in both program development and practice. The program strives to mirror genuine social norms, thus giving consumers a better understanding of how to cope with life outside the EPC.
